Advancing Whole-Person Health: Enhancing Networks of Community-Based Aging and Disability Organizations to Improve Access to Long-Term Services and Supports

ACL intends to award a single cooperative agreement to one grantee for a three-year project period. This award will support a regional community care hub (CCH) scaling strategy to expand access to long-term services and supports (LTSS) for older adults and people with disabilities as part of a broader No Wrong Door System effort. Domestic public or private non-profit entities including state and local governments, Indian tribal governments and organizations (American Indian/Alaskan Native/Native American), faith-based organizations, community-based organizations, hospitals, and institutions of higher education. Foreign entities are not eligible to compete for, or receive, awards made under this announcement. Faith-based and community organizations that meet the eligibility requirements are eligible to receive awards under this funding opportunity announcement.
